Light Up A Life Dedication Form

There are two ways you may choose to show your support:

1. Make an online donation - please follow these steps:

a) compete the information below
b) click 'submit' (clicking on this button will take you to online donation page)

If you omit to enter your dedication before clicking 'submit' just call on 08456 128812 or email info@thameshospicecare.org.uk and leave the information you would like to be added.

2. Send your donation and dedication by post. Please click on the Dedication Card link below to download and print a card. Please send the completed card to the address shown together with your donation.

	I am a UK income or capital gains tax payer and wish this donation and any I have made since 1st April 2005 and all future donations to be treated at Gift Aid until I notify you otherwise.
	To qualify for Gift Aid what you paid in income tax or capital gains tax must equal the amount we will claim in the tax year.

	Use Gift Aid and you can make your donation worth more. For every pound you give us, we get an extra 28 pence from the Inland Revenue.
