Part time 22.5 hours per week - Salary range £25,000 to £29,000 (pro rata)
As an integral part of the multidisciplinary team you will be involved in assessing and meeting the needs of patients, their families and carers. This will involve maintaining external links with the statutory, health and voluntary sectors.
You will hold a professional Social Work qualification with current GSCC registration and will demonstrate knowledge/experience of understanding and addressing issues faced by patients and families experiencing a life limiting illness. You will have excellent interpersonal and communication skills, be motivated and compassionate. Working within a psychosocial model you will have the ability to work independently as well as part of a team.
Post qualification experience is desirable but not essential.
