We are often asked 'how are patient's referred to the service?' There are several ways but mostly patients are referred by any health or social care worker, for instance your GP, District Nurse who feels the person would benefit from additional palliative care support. They would need to complete a referral form or you can refer yourself. Copies of the referral forms can be obtained by calling 08456 128812, ask to be referred to the Hospice at Home team.
Is everyone with a palliative care need, eligable for this service? Not necessarily, but just give us a call on 08456 128812 to find out if your personal circumstances would enable us to help you.
