Patient and Family Services

Providing practical and emotional support to patients and their families both up to and following bereavement is provided by the Patient and Family Services team.

The Patient Services Co-ordinator works closely with GP’s, District Nurses, local hospitals and Macmillan Nurses to arrange admission for both the In-patient and Day Support Units.

Social Workers are available to assist with various issues that include liaising with voluntary and statutory agencies for discharge planning and supporting patients and families both practically and emotionally.
